Liability Form Instructions
Once you have completed your open water training you will be required to fill out the following three forms frequently:
HSA International Activity Registration Form
HSA International Inherent Hazards AND Risks of Diving Activities
HSA International Liability Release and Express Assumption of Risk Agreement
The Activity Registration Form section can be filled out in advance of the diving activity. The PDF document that holds these three forms allows for this portion to be filled out digitally.
However the other two forms, the “Inherent Hazards AND Risks of Diving Activities” and the “Liability Release and Express Assumption of Risk Agreement” must absolutely not be filled out ahead of time. You must read them prior to filling them out. If you are sight-impaired, you should listen to an audio recording of the documents in the presence of a member of the professional staff of Freedom at Depth. You must make sure to ask questions to the staff if you are not 100% clear on the meaning of any part of these documents. Once you are satisfied that you understand the documents you must sign them in the presence of a member of the professional staff of Freedom at Depth.
