Le document PDF contenant les trois formulaires permet de remplir num\u00e9riquement ce premier formulaire.<\/p>\n\n\n\n<p class=\"has-ast-global-color-2-color has-text-color has-medium-font-size\">Toutefois, les deux autres formulaires, \u00ab LES RISQUES ET PE\u0301RILS INHE\u0301RENTS AUX ACTIVITE\u0301S DE PLONGE\u0301E \u00bb et \u00ab ENTENTE D\u2019EXONE\u0301RATION DE RESPONSABILITE\u0301 ET D\u2019ACCEPTATION EXPRESSE DES RISQUES \u00bb ne doivent absolument pas \u00eatre remplis \u00e0 l\u2019avance. En effet, ces formulaires devront \u00eatre lus et remplis en pr\u00e9sence d\u2019un membre du personnel de Libert\u00e9 en Profondeur. Si vous \u00eates malvoyant, vous devez \u00e9couter un enregistrement audio des documents en pr\u00e9sence du personnel de Libert\u00e9 en Profondeur. Vous devez vous assurer de poser toutes vos questions au personnel afin d\u2019\u00eatre \u00e0 100% certain de la signification de toutes les parties de ces documents. Une fois que vous \u00eates certain de comprendre la totalit\u00e9 des documents, vous devez les signer en pr\u00e9sence d'un membre du personnel de Libert\u00e9 en
